CASE MANAGEMENT
Liaise and work with residential rehabilitation facilities or hospitals to facilitate a safe and timely discharge to your home in the community.
Monitor and review all support and interventions to ensure they continue to meet your needs.
Research and assist in acquiring specialist equipment, including a vehicle.
Produce regular case management update reports to inform all relevant parties of progress.
Sourcing and management of a multi-disciplinary team.
IMMEDIATE NEEDS ASSESSMENT
An Immediate Needs Assessment (INA) is a report focussing on the impact of the injuries sustained on all areas of the client’s life, including personal care, leisure, transport, mood and work-related issues.
The report details the client’s goals for rehabilitation and the recommendations of the Case Manager for services, treatment and equipment in order to support the achievement of these goals.
We give an overview of likely costs for rehabilitation intervention inclusive of, and observing, the statutory and private options available if applicable along with case management hours over a set period.
ASSISTANCE WITH RECRUITMENT OF SUPPORT STAFF
Assist with the recruitment, selection, training, supervision and management of directly employed support workers to work with you wherever you are living so that you have a bespoke package of care and support.
Alternatively, research a care agency and work together to provide appropriate agency support workers to deliver your care.
Dedicated ‘on-boarding liaison’ person to guide and assist support workers through the recruitment process. Completion of all due diligence such as enhanced DBS, identity and right-to-work checks and the obtaining of two references.
Organisation of shadow shifts and interviews.
RESEARCH AND APPOINTMENT OF THERAPISTS
All therapists that work with LODCCM and our clients, go through a due diligence process to make sure that they are safe and competent to provide services to our clients. This includes checks on valid professional registrations with regulatory bodies, DBS checks and appropriate insurances. We also undergo an annual revalidation process for all therapists / therapy practices to maintain up to date and relevant data.
ASSISTANCE WITH PROPERTY ADAPTATIONS
Locate an appropriate rental property and/or property to purchase and advise on property adaptations as required.
